Care & Disease Management

Our Care and Disease Management programs can help you learn about your health and how you can better manage your health conditions. Our goal is to make sure you have the right tools to stay as healthy as possible. These programs are at no cost to you.

To learn more or to talk with a staff member, call Member Services at the phone number at the bottom of this page. You can self-refer to Care Management when you call or through your TrueCare MyLife account at MyLife.MSTrueCare.com.

Our staff can work with you one-on-one to help coordinate your health and non-health care needs. They may call you:

  • If your doctor asks us to.
  • If you ask for a phone call.
  • If our Care Management staff feels their services would be helpful to you or your family.

Our Care Management program uses information we have about a member’s health to group members into low, medium and high health risk levels. Medium and high-risk members get extra care management support to help them manage health conditions, such as:

  • Diabetes and prediabetes
  • Asthma
  • Hypertension
  • Obesity
  • Attention deficit disorder
  • Congestive heart disease
  • Organ transplants
  • Behavioral health conditions
  • Substance use disorders
  • Perinatal conditions
  • Cancer

We also support members receiving foster services. We can work with you if you need help figuring out when to get medical care from your doctor, an urgent care center or the emergency room.

We also encourage medium and high-risk members to choose a Patient-Centered Medical Home (PCMH) to coordinate their medical care. You can find these providers in our Find-A-Doctor online search tool.

Disease Management

Call 1-844-542-2610 (TTY: 711) to learn more about Disease Management (DM). This program is at no cost to you. Our DM program can help you:

  • Learn about your health and how you can care for yourself.
  • Adopt a healthy lifestyle.

Work with your doctor to better manage your health conditions. We offer DM programs for:

  • Asthma
  • Attention Deficit Disorder
  • Behavioral Health Conditions
  • Cardiovascular Diseases
  • Chronic Kidney Disease
  • Congestive Heart Disease
  • Diabetes
  • Perinatal Conditions
  • Prediabetes
  • Pregnancy
  • Obesity
  • Organ Transplants
  • Serious and persistent behavioral conditions
  • Substance Use Disorder

If your doctor, pharmacy or other health care provider tells us that you would benefit from one of these programs, we will send you information. You can also ask to be in our Disease Management programs.
