Primary Care Provider (PCP) or Patient Centered Medical Home (PCMH)
All TrueCare™ members should have a primary care provider (PCP) or patient centered medical home (PCMH) to lead their routine medical care. Your PCP is an individual physician, physician group practice, advanced practice nurse, or advanced practice nurse group trained in family medicine (general practice), internal medicine, or pediatrics. Your PCP will:
- Work with you to direct your care.
- Treat you for most of your health needs.
- Send you to a specialist, if needed.
- Admit you to the hospital.
Going to the same PCP each time you need care will help them get to know you and your health needs.
A PCMH should be used for members with more complex health needs or conditions. A PCMH provides all of the same services as a PCP, but they are more involved in coordinating care between specialists and typically have multiple specialties on site.
Choosing a PCP or PCMH
If you haven’t chosen a PCP or PCMH yet, we have ways you can find and choose one. You can login to your TrueCare MyLife account at MyLife.MSTrueCare.com (Coming Soon). Go to the Get Help tab and click on Resources to access Find a Doctor. You can also use our online Find a Doctor tool at findadoctor.MSTrueCare.com (Coming Soon) or call Member Services.
If you are a new patient for your PCP, you should call to schedule a visit. This will help your PCP get to know your health needs right away. You should go for your first visit within 90 days of joining TrueCare.
Telehealth
Telehealth uses your phone, computer, or tablet to speak to a provider. It removes the stress of needing transportation to and from the provider’s office. You can talk to a provider from wherever you are.
You can use telehealth for issues like sinus infections, allergies, rashes, and more. It also gives you quick medical advice. This can help prevent your condition from getting worse. Your providers may offer telehealth. Check with them to find out if they offer it.
Member Services: 1-833-230-2050 (TDD/TTY: 711), Monday through Friday, 7 a.m. to 8 p.m. Central Time (CT).
