Care And Disease Management

TrueCare offers care and disease management that can provide a broad spectrum of educational and follow-up services for your patients.

You can access care plans online through the provider portal to provide input into your patients’ care plans and consult with care managers.

Care Management Program

TrueCare’s Care Management program fully integrates physical health, behavioral health, functional needs, and social determinants of health. Our Integrated Care Management program connects members and their families and/or caregivers with a care team to develop an integrated care plan to address all their needs. It combines population health management with a holistic member-centric approach to deliver services across the continuum of care.

Our one-on-one personal interaction with outreach specialists, nurse care coordinators, and social workers helps support your TrueCare patient through the following:

  • Initial and ongoing assessment activities
  • Coordination of care
  • Education to promote self-management
  • Providing additional community resources

Our Care Management program also uses information we have about a member’s health to stratify members into low, medium and high health risk levels. Members are assigned appropriate risk levels based on presence of conditions of interest, as well as utilization and member needs. Members with the following conditions will be placed in medium to high-risk categories: diabetes, prediabetes, asthma, hypertension, obesity, attention deficit disorder, congestive heart disease, organ transplants, behavioral health conditions, substance use disorders, perinatal conditions, and cancer. We also support members receiving foster services.

Medium and high-risk members receive extra care management support to help them manage their health conditions. In addition, we encourage medium and high-risk members to choose a Patient-Centered Medical Home (PCMH) to coordinate their medical care. Disease Management Program

Members with certain chronic conditions will be automatically enrolled into the enhanced disease management program. If they choose not to participate in the disease management program, they may opt out by calling 1-844-542-2610.

We offer individualized education and support for many conditions and needs, including:

Members enrolled in a program will receive free information to help them better manage their condition and care options to discuss with their provider(s).

Members identified as high risk will have a nurse assigned to their case to help educate, coordinate and provide resources and tools to help the member reach their health care goals.

Our free disease management program helps our members find a path to better health through information, resources and support. We help our members through:

  • The MyHealth online well-being tool allows members 18+ to set wellness goals, get daily health tips and participate in topic-specific Journeys to improve their health.
  • The Interactive Health Library is a member-facing self-service website. It contains a variety of topic-specific articles and interactive tools such as quizzes to help members learn more about their condition(s), promote healthy habits, reduce stress and encourage emotional well-being.
  • MyStrength is an online tool for members to better their mind, body and spirit.
